Dress shirts come in more sizes than just small, medium, or large. For the best fit, know your collar size and sleeve length. This is how to measure each:

Collar Size
Using a flexible measuring tape, measure around your neck at the base of your Adam's apple and add half an inch. As a general rule, you want to be able to snugly fit one finger between your collar and neck.

Sleeve Length
Using a flexible measuring tape, measure from the base of your neck on your backside to the outside tip of your shoulder, to the outside bend of your elbow, and finally to right below your wrist or where you want the cuff to end. The sleeve, when unbuttoned, should fall a little past your wrist.

The corresponding suit jacket sizes are typically quite accurate, but not an exact science; it is there to give an approximation of size.
